When making an appointment please let the Medical Secretary know your reason for visiting.
It helps to ensure enough time is allocated. Call atleast 24 hours in advance to cancel your
appointment. We will use the time to see patients with sudden illness.
If you have an urgent medical problem or an emergency, go to a Hospital Emergency Room.
Tell them you are a patient of Shkagamik-Kwe Health Centre and give them the name of
your doctor.
Please call before you run out of medication, if you think you will need more.
Please bring your Ontario Health Card and/or Indian Status Card with you on each visit.
The health centre strives to provide continuity of care with a primary N.P. and/or M.D.
however, in the event of urgent health issues, you will be booked with the next available health care provider.
